The collaboration will integrate the GUIDE (Guiding an Improved Dementia Experience) model, a CMS Innovation initiative supporting patients and caregivers living with dementia, into CCM and PCM care plans. By aligning operational and clinical teams, the partnership will streamline coordination and enhance outcomes for qualified patients. The partnership alleviates some of the burden on practices in providing care to dementia patients by bringing new resources and expertise into the care team.
